MyHealthFinder™ is designed to help identify and locate community resources for healthy living. It is especially designed to serve the needs of people with chronic illness, including cancer survivors. You and your family, as well as your caregivers, patient navigators, and Promotores/Community Health Workers can use MyHealthFinder to locate, get directions to, and provide ratings for community support organizations and services for a healthier life.
