This mobile app was developed by the Academy for Professional Excellence's Public Child Welfare Training Academy as part of the Common Core 3.0 After 18 eLearning and the Transition Block.

The content included in this app refers to eligibility and participation requirements, re-entering care guidelines and responsibilities, links to the CDSS website for county contacts for working with young adults (the legal term is non-minor dependents or NMDs) and county child abuse hotline numbers. In addition. it contains resources and services for working with transitioning youth and young adults. Please consult with your supervisor or agency policies and procedures with regard to working with transitioning youth and young adults.
